We strive to provide quality care to our patients and their families. We welcome all new and returning patients to the practice. Initial visits and yearly visits can take up to two hours, and in most cases, your eyes will be dilated.
We make every effort to honor your appointment time, however, due to emergencies and critically ill patients, we may occasionally run late. Our staff will notify you when you check in if your appointment will be delayed. You may choose to stay and wait or reschedule.

Most medical insurances do not consider a refraction, the procedure to determine the prescription, medically necessary or a component of the complete exam. When scheduling your appointment, please let our staff know you are interested in scheduling this procedure. A charge of $46.00 will be collected at the time of service for all Medicare patients and many private insurance carriers.

FINANCIAL AND INSURANCE OBLIGATIONS
Our office is happy to file primary and if applicable, secondary insurance claims for visits and treatments of patients who have filled out their paperwork completely and have provided their current insurance card(s) at each visit. We accept most medical and vision insurance. Patients are financially liable for co-payments, co-insurance, deductibles and all charges not covered or allowed by insurance. Patients are also responsible for knowing about their insurance benefits and whether they are in-network. Co-payments are due at check-in. All other financial obligations are due upon receipt of our statement.
Patients are responsible for obtaining all insurance-required referrals from their family doctor. We recommend you call prior to your appointment, as we must have a valid referral prior to your service.

Prescription refills are processed for non-expired prescriptions within 24 hours. When you call for a refill, have the following ready: your pharmacy phone number, the name of the medication(s), and the dosage(s).
REQUESTS FOR CHARTS AND MISC PAPERWORK
Requests for charts, copies, or transfers will be completed within two weeks of a written request. Generally, for accounts in good standing, the minimal preparation fee will be waived. Requests for paperwork for medical leave and disability will be completed in a timely fashion; generally within two weeks. All fees will be explained before processing, and payment is required before completion.
